- Transcript of records (for college level)
- form 137 or 138 for high school level
- 6 pcs passport picture
- NSO birth certificate
- NSO marriage contract
- Entrance Exam Fee/Processing Fee Php 600.00

OPTION B (Payment of tuition fee only. OJT, Red Cross Training and Medical will only be charged when the student decides to take it. Student only pays for all basic academic requirements. S/he however may not be given a license until completion of the other affiliated requirements)
